Summary
During backloading of an empty refrigerated container, a Pipe Supply Vessel lost position and the load swung uncontrollably. Wind gusts, thruster wash and loss of a thruster DP signal contributed. Nobody was harmed and equipment was undamaged. Lessons address vessel-to-vessel operational limits, cargo-lifting procedures and adherence to ASOGs.
