Summary
A crane wire failure during recovery of a divers’ small boat caused a block to fall 13 m, narrowly missing a crewman. Inadequate maintenance and inspection were identified. Recommendations address freshwater washing of submerged wire, protective coatings, consideration of additional inspections and conditional restrictions on personnel remaining aboard during launch or recovery.
