Summary
A changed berth prompted recovery of a vessel’s partly deployed gangway. Two crew members removed securing pins without first relieving lifting-wire tension. Crowbar pressure caused the outboard pin to jump out and a gangway section to trap their legs. One suffered a serious fracture; the flash stresses following recovery procedures.
