Summary
During an annual rescue-craft and davit test aboard a vessel in dry dock, an unreleased lashing caused the craft to tip into a railing and its stern to become stuck. Hydraulic piping burst under abnormal pressure. Nobody was injured. The flash recommends pre-launch checks, clearer communication and detailed launch and retrieval procedures.
