Summary
During crane brake testing aboard a vessel alongside, the boom descended uncontrollably into its crutch and a 60 kg steel sign fell 15 m to the deck. Three of four stitch welds had rusted away. Initial recommendations, pending investigation completion, addressed testing procedures, permit requirements and inspections before further use.
