Summary
A vessel drifting while awaiting pilots collided with a fishing vessel despite repeated lookout warnings. No crew injuries were reported. Findings identify failures to follow collision regulations and standing orders, ineffective drift-position risk assessment and watch-handover distraction. Preventative measures address watchkeeping, monitoring approaching vessels and deferring handovers when navigation safety is affected.
