Summary
During flow-line installation on an oil platform, displaced plastic grating left a gap into which a rigging crewman slipped without injury. Loosened securing clips from platform flexing were assumed to explain the failure. Actions included checking and securing grating, barriering the area and recovering the fallen section using an ROV.
