Summary
An offshore drilling crew was securing a rig and well before storm evacuation when a choker sling came loose during drill-collar handling. A helper slipped and was fatally struck across the back by the falling collar. The operator instructed drilling contractors to use lift plugs to prevent slings or tail chains slipping off.
