Summary
Management visits identified three cases of crew-made wooden-board and rope supports for painting and repairs at height on vessels. The flash highlights poor hazard recognition, procedural awareness and inspection, alongside unresolved previous findings. Unsafe equipment was destroyed and arrangements introduced for suitably designed, certified fall-arrest and man-riding equipment.
