Summary
Two employees undertaking a construction weld repair were killed when a curved gas export pipe fell approximately 60 cm from temporary wooden supports. The flash highlights unrecognised dropped-object hazards and lateral movement associated with thermal expansion, differential heating and residual pre-stress, despite rigorous application of permits to known risks.
