Summary
Orade struck the Apex Light tower and grounded at the Ouse–Trent confluence during pilotage. The investigation identified poor master–pilot communication, probable selection of autopilot instead of riverpilot, inadequate monitoring and awkward bridge controls. Probable master fatigue was also identified; recommendations addressed fatigue, bridge team management and ergonomics.
