Summary
MAIB investigates Key Bora’s contact with a Hull jetty following inadequate controllable-pitch propeller astern response. Timed tests identified delay in the closed-loop control system. The report examines commissioning parameters, missing performance standards, pilot testing, backup-control familiarity and communication difficulties, and recommends corrective work, propulsion-failure drills and backup-system testing.
