Summary
Investigation of Coelleira’s grounding at Ve Skerries, Shetland, and subsequent loss following unsuccessful salvage. It examines inadequate passage planning, ineffective lookout and position monitoring, electronic navigation limitations and possible fatigue. All 15 crew were evacuated without injury.
