Summary
Investigation of BORGA’s grounding under pilotage at Milford Haven examines delayed removal of port helm and speed-dependent manoeuvring with a controllable-pitch propeller and Becker rudder. Simulator reconstructions supported the significance of astern pitch. The report also analyses steering coupling failure, bridge monitoring and hydraulic alarm limitations; no oil pollution occurred.
