Summary

An able seaman died following cold-water immersion alongside Glasgow, having almost certainly fallen while accessing Joanna’s hatch-cover straddle lift. The investigation examines unsafe platform access, probable alcohol impairment, worn protective equipment and delayed recovery. It identifies weaknesses in risk assessment and safety management, and records corrective action without further recommendations.
