Summary
Investigation of an accidental BOP wellhead disconnect on Ocean Saratoga during completion operations. Crossed Yellow Pod hoses, deficient procedures and missed verification opportunities allowed the fault to remain undetected. The incident released calcium bromide completion fluid.
