Summary
BSEE examines a hydrocarbon spray during removal of a well equalisation header on the Titan spar platform. Residual fluid and trapped pressure in a dog-leg were overlooked despite isolation. Gas detection prompted muster; neither worker was injured. The report describes protective equipment, medical assessment and planned reviews of residual-pressure hazards.
