Summary
Investigation of a platform fire during choke removal that injured two contractors. Apparent inadvertent opening of the surface safety valve released well fluids through the open choke body; the generator was assumed to ignite the gas. Recommendations address manual isolation, choke relocation, flow-line changes and fire shielding around living quarters.
