Summary
An offshore production platform suffered a gas fire after a choke cap detached during restart. A generator ignited the escaping gas. Fire-loop shutdown left a manually locked-open valve unclosed; a wellhead safety valve subsequently stopped the supply. No injuries and minor damage were reported, with manifold rebuilding planned.
