Summary
BSEE investigates a rigger’s injury during a blind crane lift to place a wireline riser and gate valve assembly into a cargo basket. The shifting load trapped him against the basket. Findings identify congested positioning, unrecognised pinch-point exposure and failures to apply existing safe-work instructions and stop-work authority.
