Summary
BSEE investigates a pelvic injury during casing offloading on the MODS 400 platform rig. A suspended bundle trapped the deck foreman against a sub-rack basket. Findings address the shrinking impact zone, combined supervision and tally duties, unheeded warnings, failure to follow the job safety analysis and unused stop-work authority.
