Summary
An investigation examines the discharge of approximately 458 barrels of synthetic-based mud into the Gulf of Mexico during transfer to a platform rig. Incorrect valve alignment and unperformed checks recorded as completed contributed to the release. The report describes crew communication, a stand-down and changes to the fluid-transfer job safety analysis.
