Summary
Investigation of a synthetic-base drilling mud discharge into Gulf of Mexico waters during casing operations and mixing-tank drainage. Incorrect valve positioning connected the mud supply to an overboard drain. The report records a calculated loss of 56 barrels and subsequent valve locking, permit-to-work integration, hazard analysis and planned valve-position checks.
