Summary
An investigation records the discharge of 66 barrels of synthetic-based mud from Thunder Horse PDQ during seawater pumping. Shaker-screen rubber prevented a pit butterfly valve from closing fully. The report identifies manifold leak-detection limitations and absent debris screening as contributing factors; closing the master dump valve stopped the discharge.
