Summary
An investigation describes a floor hand’s facial injury during drill-pipe tripping using a top drive and tail-in rope. The pipe swung into him after the rope slipped from his hands. Investigators identified body positioning, mud-saturated rope, inadequate job safety analysis and procedural non-compliance. Medical evacuation followed; no recurrence recommendations were made.
