Summary
A driller fractured his back after falling approximately 3 feet 3 inches during assembly of BOP working platforms on the Auger TLP. The investigation identified missing access-opening protection, unused fall-protection gear and positional awareness issues. It also examined inconsistent requirements between the job safety analysis and fall protection plan.
