Summary
A derrickman aboard Noble Valiant was pinned between a pipe stand and the automatic pipe-racking arm while tying open a malfunctioning fingerboard latch. The investigation examines missing work-at-height authorisation, unsafe positioning, zone-management gaps and failures to recognise the hazard despite available CCTV coverage.
