Summary
A floorhand handling pipe on a Gulf of Mexico drillship was fatally pinned between a retracting skate’s loading platform and a stanchion inside a controlled red zone. Causes remained under investigation. BSEE recommends reviewing hazardous areas and safe practices, and considering zone marking, access training, crew discussion and equipment-layout review.
