Summary
This investigation examines oil pollution from Ship Shoal 181 B following a heater-treater dump-valve failure. Excess hydrocarbons passed through the skimmer and water clarifier into Gulf waters while production continued. Findings identify delayed shutdown as contributory and describe pump-discharge repiping and vessel cleaning to prevent recurrence.
