Summary
An investigation records approximately 4.71 barrels of oil escaping overboard from a cracked heater treater fire tube. Preliminary findings identify the crack at a sealed weld and suggest hot-spot fatigue may have contributed. Flow was shut in, the tube repaired and a containment skid installed.
