Summary
Investigation of a platform fire during hot-work removal of a production drain trough. Hot slag ignited gas vapours at an unbolted drain flange. A fire watch fractured his kneecap while jumping to a staircase. Findings address inadequate drain isolation and flushing, gaps in job safety analysis, and operator follow-up actions.
