Summary
Investigation of a floorhand’s finger amputations during wireline retrieval on the H&P 100 rig in Green Canyon. A snagged dual pack-off caused the wireline to pull free from its rope socket. Findings address hand placement, incomplete task analysis and failure to stop work, alongside subsequent planning and training changes.
