Summary
BSEE investigates firewater pipe ruptures during power restoration following an emergency shutdown at Horn Mountain. Water hammer impaired firefighting supplies and dislodged a support clamp near a muster area, without injury. Findings examine pump power interruptions, a suspected leaking check valve, inaccurate procedures and failures to purge trapped air.
