Summary
Investigation of an offshore drilling-fluid spill through a corroded two-inch transfer line routed through a rig pre-load tank. Diesel-based mud escaped through a partially open dump valve. The report identifies inadequate pre-transfer checks and preventive maintenance, and records valve closure, line rerouting and removal of fluid from the tank.
