Summary
BSEE investigated an injury during permanent well abandonment at East Cameron 346A. An unsecured vee-door gate, removed to lay down recovered casing sections, fell and struck a worker’s knee, requiring medical evacuation. Findings identified absent gate removal and storage procedures and omission of gate removal from the job safety analysis and work instructions.
