Summary
Two fires on a production platform’s glycol reboiler involved leakage from a loose flange and, after restart, flashing cold glycol vented through the reflux column. Both were ignited by uninsulated compressor exhaust. The alert records suppression, no injuries, and corrective changes to vent location, flange bolts, dump-valve adjustment and exhaust insulation.
