Summary
During plug and abandonment work, a cut pipe piece lodged below the rotary table was dislodged when casing was lifted. It fell approximately 35 feet and seriously injured a crew member on the Texas Deck. The alert recommends reviewing job safety analyses and periodically checking for loose materials near openings.
