Safety Alert 319 - Chemical Injection Point Corrosion Causes Production Header Failure

Summary

A Gulf of Mexico production header ruptured after acidic demulsifier injection caused localised corrosion beneath the injection point. Production fluid sprayed three workers and the platform was evacuated. BSEE recommends procedures for chemical injection equipment and visual and non-destructive inspection, highlighting injection quills to direct chemicals away from pipe walls.
