Summary
Two December 2019 incidents involved offshore workers stepping on unsecured or unsupported drain grating, resulting in back and knee injuries and restricted duty. BSEE recommends considering grating inspections, welding shorter sections where applicable, end caps against sliding, improved situational awareness and sufficient lighting to improve visibility.
