Summary
This reissued alert describes fourteen reported oil spills during transfers of fuel oil and oil-phase drilling fluids. Approximately half were attributed to hose failure and half to inattentive personnel. It recommends prompt hose inspection, replacement of defective hoses, proper storage without sharp bends, and visual monitoring at both transfer stations throughout operations.
