Summary
An offshore platform crane detached from its pedestal and fell into the water during offloading of a mud tank, killing the operator. Investigation attributed failure to overload, finding no brake, clutch or material defects. The operator’s actions addressed load monitoring, fluid-containing open tanks, training and heavy-lift procedures.
