Summary
A casing joint approximately 40 feet long fell through collar elevators as it approached vertical during rig-floor hoisting. Despite insertion of the safety pin, the joint fell, hit the V-door and slid to the catwalk, striking a floorman’s lower back. He was taken to hospital with a back injury.
