Summary
A near miss during maintenance of a 13 5/8-inch 10K annular blowout preventer involved sudden cap ejection while a second pull-down bolt was being removed. The cap rose 4–5 ft, landing slightly off centre on the assembly. Three remaining bolts sheared; no personnel were injured.
