Summary
During testing of a subsea BOP stack, the blue pod’s shear accumulator failed to charge and a faulty DDV was suspected. Its solenoid housing was tilted, with four bolts sheared. Removing the remaining two bolts to investigate caused the housing top to fly into a bulkhead.
