Summary
During hammering of 30-inch drive pipe through hard formation, the pipe fell approximately 10–15 feet before contacting elevators. The driving swage collapsed and was ejected, striking rig structures. No injuries occurred; the alert credits the crew’s job safety analysis and pre-job meeting with contributing to that outcome.
