Summary
During casing running, a crew extended a fill line using a Kelly valve because suitable joints were unavailable. An air hoist line, chain sling and rig tong supported the arrangement. Top-drive make-up torque moved the arrangement and tong towards a worker, trapping him against a spider elevator and fracturing his right arm.
