Summary
During withdrawal of a bottom-hole assembly, a hydraulic jar accelerator caught beneath the inner bowl, lifting the master bushing approximately 6–8 inches. A worker lost his footing and his foot moved beneath the bushing as it fell. His steel-toe boot was damaged and two right toes were amputated.
