Summary
An alert describes unauthorised sludge-tank entry after high-pressure washing failed to clear a barite-blocked flow line. A derrickman entered with a rod, cleared the obstruction, then slipped on mud while leaving and struck a ladder, severely bruising his chest. Medical examination found him fit to resume normal duties.
