Summary
This alert describes three incidents during pipe handling and tripping out. A loose hydraulic hose caused elevators to open; premature opening dropped a pipe stand onto the rig floor. After jarring, a hydraulic-fitting cover came loose and fell towards the floor, but the derrickman caught it.
