Summary
During rig substructure dismantling, a crane rigger stepped onto a rotary beam to reposition unevenly centred lifting chains after the beam bound. The beam came free and fell, and the rigger fell approximately 30 feet (9m) onto matting boards, sustaining fatal injuries.
